What to Expect
At your first appointment.
Please complete your registration and health history forms from home prior to your first appointment, or arrive at least 15 minutes early to complete them in office.

A list of medications
Your first appointment will be a consultation and exam, where we will review your dental and medical history, your treatment needs, and address any questions or concerns you may have. If we need x-rays, we will take them at this time.
All patients under the age of 18 will need to be accompanied by a parent or legal guardian.

We will submit the claim to your insurance company on your behalf and provide you with an estimated out of pocket cost prior to treatment. Your insurance company will determine the amount of coverage and the difference in cost is your responsibility to pay.

Cancellations
We require at least 48 hours notice to avoid cancellation fees.
